Clinical-pathological agreement and Goldman's classification as a quality index in neonatology

Abstract

Introduction: The agreement between the clinical diagnosis and the pathological outcome is an indicator of the quality of medical care.

Objective: To determine the relationship between clinical-pathological agreement and variables related to neonatal death.and Goldman's classification as a quality index in neonatology.

Methods: Retrospective, descriptive, cross-sectional study with newborns who died at "Dr Ángel Arturo Aballí" Maternal and Children Hospital from 2009 to 2019. The universe consisted of 74 deceased newborns and the sample of 65. The autopsy index, sex, birth weight, gestational age, type of delivery, age at death, direct causes of death and concordance between pathological findings and clinical diagnoses were determined.

Results: The autopsy rate was 90.5 %, underweight neonates were 4.4 times more likely to agree (CI: 1.33-14.74). Greater agreement was observed between clinical and pathological diagnoses in congenital pneumonia with 30.5%, hypoxia and perinatal asphyxia with 23.9% and congenital malformations, deformities and chromosomal abnormalities 19.6%. Cases classified as V class prevailed in 63.1% and 20.0% in I class according to Goldman classification.

Conclusions: Low birth weight is the variable that shows the highest probability of clinical-pathological agreement. Patients with total agreement between clinical and pathological diagnoses predominate, although there are a considerable number of cases with major discrepancies in the main diagnoses.
