Abstract

Introduction: The detection of chylothorax in the pediatric population leads to study to rule out mediastinal disease, whose frequent origin is lymphomas and tuberculosis at the pulmonary level.

Objective: To examine a patient with mediastinal non-Hogdkin lymphoma that clinically manifested as respiratory distress plus unilateral chylothorax.

Case presentation: Female patient aged 1 year and 5 months, who jhad an onset of sudden respiratory distress. X-ray showed right pleural effusion. Diagnostic thoracentesis was performed with subsequent pleural drainage. Chylothorax was demonstrated. CT scan and surgical biopsy confirmed the diagnosis of non-Hodgkin lymphoma. The patient evolved unfavorably and died 10 days after presenting the first clinical manifestation.

Conclusions: The presence of chylothorax in the pediatric population requires the search for mediastinal disease that points to neoplastic or infectious symptoms as the first possibility.
