Infantile Hepatic Hemangioma, An Unexpected Diagnosis in the Context of Neonatal Sepsis

Abstract

Introduction: Hepatic hemangiomas are the most common benign liver tumors in childhood. Their presentation in the immediate neonatal period is rare and can be confused with severe infectious processes, complicating initial management.

Objective: To describe the case of a neonate with apparent sepsis, whose imaging study revealed a liver lesion suggestive of hemangioma.

Case report: The patient presented with fever, leukocytosis with neutrophilia, and a positive urine culture for Escherichia coli. An abdominal ultrasound, performed as part of the workup, identified a complex hepatic mass, sonographically compatible with infantile hemangioma on contrast-enhanced abdominal computed tomography. Management combined antibiotic therapy for sepsis, treatment, and expectant observation for the vascular lesion.

Conclusions: In the presence of septic findings in the neonate, the coexistence of structural pathologies should be considered. Hepatic hemangioma, although uncommon in the first few days of life, may be an incidental finding that requires a multidisciplinary approach for its correct diagnosis and conservative management.
