Breastfeeding and pain response during heel prick in late preterm newborns

Abstract

Introduction: Neonatal pain associated with invasive procedures is a common problem in neonatal intensive care units. Breastfeeding has been proposed as an effective non-pharmacological analgesic measure to reduce the pain response during minor procedures.

Objective: To evaluate the association between breastfeeding and the pain response during heel prick in late preterm newborns.

Methods: A multicenter, prospective, analytical study with a non-randomized control group was conducted in 342 late preterm newborns admitted to neonatal intensive care units in Artemisa and Havana, between January 2020 and December 2021. An intervention group was formed, which received breastfeeding before and during the heel prick, and a control group, without intervention. The pain response was evaluated using the Premature Infant Pain Profile scale, the duration of crying, and physiological alterations. Relative risks, 95% confidence intervals, and the chi-square test were calculated.

Results: Newborns without breastfeeding presented a higher frequency of heart rate alterations and pain scores ≥ 8 on the Premature Infant Pain Profile scale. They also showed a higher frequency of prolonged crying and alterations in mean arterial pressure, although these associations did not reach statistical significance.

Conclusions: Breastfeeding was associated with a lower frequency of heart rate alterations and high pain scores during heel prick in late preterm newborns.
