Trichobezoar in a 9-year-old girl

Abstract

Introduction: Bezoar is a conglomerate of hair impacted in the stomach that occurs more frequently in patients with mental disorders (trichotillomania) and represent a rare entity that can lead to diagnostic and therapeutic difficulties.

Objective: To describe, due to its rare frequency, a pediatric case of gastric trichobezoar, accompanied by trichotillomania and trichophagia.

Case presentation: A 9-year-old female patient with no apparent pathological personal history, admitted due to epigastric pain, food vomiting and a palpable tumor in that area. Clinical examination showed areas of alopecia and a mobile, painful epigastric tumor, rounded between 7 and 10 cm. The upper digestive endoscopy allowed to visualize a giant trichobezoar of approximately 17 cm, visible from the subcardial region to the antral region, which required a minimal left subcostal laparotomy, followed by gastrotomy with evacuation of the gastric trichobezoar. Post-surgical evolution was favourable.

Conclusions: Gastric trichobezoar is a rarity whose diagnosis should be considered in pediatric female patients with chronic dyspeptic symptoms accompanied by a history of areas of alopecia and trichotillomania. The management of this condition often requires surgical intervention and a multidisciplinary approach as most of these patients suffer from mental disorders.
