Bacterial Resistance and Detection of Β-Lactamases in Children Admitted for Urinary Tract Infection

Abstract

Introduction: Urinary tract infection in children is a health problem. Currently, resistance to Escherichia coli, the main pathogen in this disease, is increasing worldwide.

Objective: To describe the behavior of antimicrobial resistance in vitro in the causal agents of urinary tract infection in children.

Methods: A cross-sectional descriptive observational study was carried out in the Microbiology laboratory of William Soler Ledea Pediatric Hospital, during 2021 - 2022. The sample included 342 positive urine cultures with a count of more than 105 CFU/mL corresponding to children admitted with clinical diagnosis of urinary infection. The identification of the germ was carried out by conventional biochemical tests. Antimicrobial susceptibility was evaluated by Kirby-Bauer method and the production of extended-spectrum β-lactamases was determined.

Results: Escherichia coli represented 76% of the isolates. In the total number of isolated germs, resistance was detected to: aminopenicillins and third-generation cephalosporins (30%), ciprofloxacin (20.8%), norfloxacin and sulfaprim (17.3%), gentamicin (8.2%) and amikacin (3.5%). Fosfomycin had only 4.7% resistant isolates. 21% of the total isolates were producers of extended-spectrum β-lactamases. Escherichia coli was the largest producer with 74%.

Conclusions: The results suggest that the causal agents of urinary infection in children present problems of resistance to beta-lactams that are frequently accompanied by resistance to other pharmacological groups, this constitutes a health problem revealing the need for continuous monitoring of the antimicrobial susceptibility of these pathogens.
