Abstract

Introduction: Non-alcoholic fatty liver disease is the most common cause of chronic liver disease in childhood, adolescence and adults.

Objective: To analyze the role of the gut microbiome in non-alcoholic fatty liver disease.

Methods: Publications in Spanish and English were reviewed in PubMed, Google Scholar, SciELO, between January 2010 and-December 2020, and the terms used were: intestinal microbiota, fatty liver disease, non-alcoholic steatohepatitis, dysbiosis and intestinal microbiota modulation.

Analysis and synthesis of information: The importance of the impact of non-alcoholic fatty liver disease on health and its association with obesity in adults and children, the pathophysiological mechanisms of the intestine-liver axis and the effects of dysbiosis of the microbiota, barrier and intestinal permeability are raised. Probiotic therapeutic manipulation is analyzed as a new adjuvant therapy.

Conclusions: There are arguments about the dynamic changes that occur in the intestine-liver axis, an interrelation that intervenes in a key way in the pathogenesis of non-alcoholic fatty liver disease, especially steatohepatitis. The participation of this axis in the dysbiosis of the intestinal microbiota with all its genetic material (microbiome) and in the increase of the permeability of the intestinal epithelium associated with the microbial composition is highlighted. These aspects are not fully clarified. The use of next-generation probiotics to modulate the gut microbiota offers proven promising results in animal experimentation and in still limited randomized controlled clinical trials.
