Diagnostic approach to the finding of hyperphosphatasemia in a routine analysis

Abstract

Introduction: Hyperphosphatasemia is a marked elevation of alkaline phosphatase, generally an incidental finding in routine analysis.

Objective: To examine the diagnostic approach in the finding of hyperphosphatasemia in a routine analysis.

Methods: The national and international literature of the last 25 years was reviewed in the databases PubMed, Medline, Scopus, SciELO, Elsevier, JAMA and other open access journals, and in the Google Scholar search engine. The following keywords were used: "hyperphosphatasemia", "elevated alkaline phosphatase" and "alkaline phosphatase", and documents in Spanish and English. The quality, reliability and validity of 38 selected articles were analyzed, of which 25 were referenced.

Analysis and synthesis of information: The aspects of etiology, diagnosis and differential diagnosis of hyperphosphatasemia were reviewed and some considerations were made about its diagnostic approach. The causes of the disease and the drugs that produce it are described. Other forms of hyperphosphatasemia occur.

Conclusions: Benign transient hyperphosphatasemia is the most frequent cause of isolated elevation of alkaline phosphatase in the pediatric population; however, given the presence of this enzyme in different organs and tissues, the differential diagnosis includes diseases of the liver, kidney, bone, intestine and blood. The knowledge of this entity contributes to reduce unnecessary concerns and approaches.
